Medical Records Request

In response to the Health Insurance Portability and Accountability Act (HIPPA) of 1996, we strive to protect all medical records. In a effort to comply, we have policies and procedures to insure that your confidential medical records are handled in a manner meeting all necessary guidelines.

Medical records will be release only upon written request from the patient. Written requests must be in accordance with the Uniform Health Care Information Act. 

Edge Medical care will only release records that were created and maintained by our providers and clinics. We will not release records received from other clinics or providers. Please allow 2 weeks for the records to be available for you to pick up. 

To request your medical records, please fill out our medical record request sheet which will be with the front desk. Make not that you also require records from other providers or clinics
